We are here to support our communities families and seniors that are transitioning from independent life to assisted living or memory care. We help ease the stress of researching facilities by narrowing down the overwhelming options to a select few that meet your loved ones time frame, medical needs, financial budget, and social requirements. Our 3-Step Process
1. Assessment of Care Needs:

A Personal Assessment is undertaken to determine a client’s level of care, financial budget, geographic, social and emotional needs and preferences.

2. Email compatible communities:

Facility options that match a client’s profile are presented.
We review current care situations with the client to ensure the services or situation is safe for seniors and meets all needs.
We schedule tours for clients and families to selected senior living communities and care providers.

3. Guidance Through Placement

Guidance is provided through placement and transition, help is provided to facilitate a move.
Communication between communities, health care providers, and caregivers are facilitated.
Transition follow-up is provided to ensure needs and expectations are met.

WHEN TO TRANSITION TO ASSISTED LIVING OR MEMORY CARE

Where Extra Support is Provided

There are at least seven valid reasons to start considering assisted living, each holding a different value depending on your personal journey.
A common reason to consider assisted living is simply living alone, feeling lonely or depressed, and needing assistance with everyday activities.
Consider assisted living if personal care, medication management, meal preparation, housekeeping or coordinating transportation is becoming difficult to do.
Assisted living is also worth considering if you have health related issues that require supervision such as arthritis, failing eyesight, or a recent diagnosis of Alzheimer’s. More advanced Alzheimer’s Memory care is a more appropriate choice depending on behaviors and elopement risks.
Assisted living or Memory Care may be the best option for you, especially if your safety and wellbeing are at risk.
